How to submit Application Form 怎样交上表格
We deeply hope that you could download and fill in the form via through your computer and send along with the RM50.00 (via bank transaction , please send your proof of payment example like receipt))
to wonglihyen@yahoo.com or Keman11@gmail.com
Or
If you can not pass the form via online, please send the application form and the RM50.00 (via bank transaction , please send your proof of payment example like receipt) to 7th Floor, KEMAN Secretary, MCA Youth, MCA Building ,Jalan Ampang , 50450 Kuala Lumpur.
** Do not send RM50.00 cash via post

How to submit Camp Fees(RM50) 怎样交上营费（五十零吉）
报名费： 50令吉 (包括膳食、住宿、讲义、营服、交通及保险等)Fees: RM50 (Including Meals, Accommodation, Notes, T-Shirt, Transportations, Insurance & etc)
付款方式：
(1) 银行汇款或支票 (请在背后注明申请者姓名、身份证号码及联络电话)
Payment: Bank Payment OR Cheque (Please write the applicant’s Name, IC No & Contact No at the back)

抬头 Pay to: MCA Youth HQ A/C 2

(2) 邮政汇票 (Wang Pos)抬头 Pay to: MCA National Youth
(3) 现金交至马青总团 Pay by cash at MCA National Youth HQ, 7th Floor, Wisma MCA, 163 Jalan Ampang, 50450 KL
***请勿寄现金，以免遗失***Do not send cash by mail
Once you have done on the transaction. Kindly send your proof of payment(example like receipt
(photostatecopy is acceptable)) to keman11@gmail.com or wonglihyen@yahoo.com with you reference number.
